個人資料蒐集、處理及利用告知聲明 財團法人國家衛生研究院(以下簡稱本院)因蒐集、處理及利用個人資料，謹依個人資料保護法(以下簡稱個資法)，第8條及第9條規定，向您告知以下內容，敬請詳細審閱。
1、蒐集目的：本院基於COVID-19防疫及事後追蹤需求，必須取得您的個人資料。
2、蒐集個人資料之類別
辨識個人者(姓名、手機號碼)
3、個人資料利用之期間、地區、對象及方式
期間：衛生福利部疾病管制署公告之COVID-19防疫期間。
地區：台灣地區(包括澎湖、金門及馬祖等地區)。
對象：本院及相關主管機關。
方式：電子文件、紙本或其他合於當時科技之適當方式。
4、個資當事人得依個資法第三條規定請求查詢、閱覽、製給複製本、補充或更正、請求停止蒐集、處理或利用及請求刪除，其行使方式依法令及本院相關規定。
5、個資當事人不得拒絕提供資料，並應確認資料正確性，如未提供個人資料，不得進入本院。
6、本院得依法令或遵照主管機關、司法機關依法所為之要求，提供個人資料及相關資料。
7、本網將使用Cookie至於手機中，以提供最便利的「ONE DAY PASS」一日通行證取得方式。

(3)Target: This court and related authorities.
(4)Method: Electronic file, paper or other appropriate method in line with the current technology.
4、 Individual parties may request inquiries, read, make copies, supplements or corrections, stop collection, processing, or use, and request deletion in accordance with Article 3 of the Individual Assets Law. The method of exercise is in accordance with laws and regulations and relevant regulations of this court.
5、 Individuals concerned shall not refuse to provide information, and shall confirm the correctness of the information. If no personal information is provided, they shall not enter this court.
6、 This court may provide personal information and related information in accordance with the law or in compliance with the requirements of the competent authority and judicial authority.
7、 This website will use cookies in mobile phones to provide the most convenient way to obtain the "ONE DAY PASS" one-day pass.
